MEDICAID WAIVER SERVICES
Adults aged 19 and older with developmental disabilities can apply for services. Eligibility is initially handled through the Department of Behavioral Health and Developmental Disabilities. All individuals interested in services will need to be placed on the state planning list that is administered through the Department of Behavioral Health and Developmental Disabilities (DBHDD).
Please call the Region 5 Field Office at 912-303-1670 and ask for the Intake and Evaluation Unit.
Note: Individuals in the school system should request to be placed on the planning list at least 5 years before graduation.
FEE FOR SERVICE
EmployAbility accepts individuals who privately pay for services. Fees are based on the type of service, the intensity of support required, and the volume of services desired.
